   ITEMS NEEDED AT TIME OF REGISTRATION

Immunizations (2MMR’s, 2 Varicellas or proof of chicken pox)

Discipline from previous school or statement of no discipline, in a sealed envelope

Most recent grades from previous school.

The following items are not mandatory for summer registration but provide important information for appropriate classroom placement.

  Standardized test results

  Complete address and phone number of last school attended

  Proof of gifted, accelerated, and/or special education programs that your child was involved in at their prior school

*During the school year, in order to allow receipt of full school record, students new to Youth Middle School will begin class on the day following completion of registration packet.

Notes regarding immunization and other health requirements:

1. Certificate of Immunization must be provided to the middle school on form 3031, 3032 or 3189.  Forms are available from the health department or your doctor.

2. Certificate of Dental Exam (GA From 3300). This can also be obtained from the heath department or your dentist.

3. Georgia law requires that all students entering the sixth grade have the chickenpox (varicella) vaccination or proof of immunity.

4. All students entering the sixth grade must have two MMR shots after their first birthday and the shots must be at least 30 days apart.

5. Students entering any school in Georgia for the first time are required to have the Hepatitis B series of immunizations.
